My 16.4.2 has very low output levels from the main outputs, both left and right. I'm not sure about any of the aux sends or the mono out. This happened to me right before a show tonight and I didn't have time to check any other outs. The board worked fine yesterday. The board appeared to be working fine today. All my inputs showed normal levels on the meters. My output meters showed normal also. I initially thought that I had an amplifier problem. So I changed out the mains amp and the problem remained. I quickly grabbed another board for FOH and that console worked properly. An output signal is present, it's just very low in volume.

Did you check the tiny little pot next to the Main out on the back of the mixer? It could have been turned down possibly? Also, like any other pot I have had to wiggle it before on older mixers to get the volume to come back. Like an old dusty pot.

Oh yeah. That's the first thing that I checked. I did some further testing tonight. The mono output is significantly louder than either left or right out. This is with both pots (main out and mono out) on the rear at the same position, fully clockwise. The audio on the mains is clean and clear, it's just nowhere near the output level that it usually is.
